Moonshot Medical and Performance ("Moonshot," "we," "us") operates the Moonshot Clinic platform, including online appointment booking, patient portal, intake forms, and related services. This Privacy Policy describes how we collect, use, and protect your personal information.

1. Information We Collect

We collect the following types of information:

Contact information: Name, email address, phone number, date of birth
Health information: Medical history, intake form responses, lab results, and other clinical data you provide
Appointment information: Scheduling details, provider assignments, visit notes
Payment information: Billing details processed securely through Stripe (we do not store card numbers)
Usage data: IP address, browser type, and pages visited for security and service improvement

2. How We Use Your Information

We use your information to:

Schedule and manage your appointments
Send appointment reminders, confirmations, and cancellation notices via email and text message
Deliver intake forms and collect your responses before visits
Provide clinical care and maintain your medical records
Process payments and manage billing
Communicate important updates about your care
Improve our services and platform

3. Text Message Communications

By providing your phone number and booking an appointment, you consent to receive text messages related to your care, including:

Appointment reminders and confirmations
Cancellation and rescheduling notices
Links to complete intake forms

Message frequency varies based on your appointment activity. Message and data rates may apply. You can opt out at any time by replying STOP to any message. Reply HELP for assistance. For support, email info@moonshotmp.com.

4. Information Sharing

We do not sell, rent, or share your personal information with third parties for marketing purposes. We may share information with:

Healthcare providers involved in your care at our clinic
Service providers who help us operate (e.g., email delivery, text messaging, payment processing) — these providers are contractually required to protect your data
Legal authorities when required by law or to protect safety

5. Data Security

We implement reasonable administrative, technical, and physical safeguards to protect your information, including encrypted data transmission (TLS), secure database storage, and access controls limited to authorized personnel.

6. Data Retention

We retain your information for as long as necessary to provide services and comply with legal obligations, including medical record retention requirements.

7. Your Rights

You may request access to, correction of, or deletion of your personal information by contacting us. Note that certain medical records must be retained as required by law.

We may update this policy from time to time. Changes will be posted on this page with an updated date.

9. Mobile App and Apple Health

The Moonshot Clinic mobile app on iOS (and Health Connect on Android) can optionally read a limited set of data from Apple Health to auto-fill your daily wellness log. This is opt-in — you must grant permission, and you can use the app fully without granting it.

What we read (read-only):

Body weight
Body fat percentage
Whether you completed a strength workout today (workout duration only, from a whitelisted set of strength-training activity types)

What we never read: steps, sleep, heart rate, blood pressure, blood glucose, nutrition, mindfulness, location, GPS routes, or any other Apple Health data type.

What we never write: Moonshot Clinic does not write anything back to Apple Health. The integration is one-way, read-only.

How the data is used: The values are sent over TLS to your clinic's account on our platform and saved to your daily wellness log so you don't have to retype them. The data is shared only with the authorized members of your care team at your clinic under HIPAA. It is never used for advertising, marketing, analytics, profiling, or sold to third parties.

Revoking access: You can revoke Apple Health permission at any time in iOS Settings → Privacy & Security → Health → Moonshot Clinic. Revoking does not delete the values already saved to your log; to delete those, sign in and remove the daily log entry, or delete your account.

Account deletion: You can delete your Moonshot Clinic account, including all daily log entries, progress photos, and message history, from inside the app (Settings → Account → Delete Account) or by emailing support@moonshotclinic.com.
