These Terms and Conditions ("Terms") govern your use of the Moonshot Clinic platform operated by Moonshot Medical and Performance ("Moonshot," "we," "us"), including online booking, the patient portal, intake forms, and text message communications.

By providing your phone number through our booking portal or patient intake process, you consent to receive automated text messages from Moonshot Clinic, including:
Message frequency varies based on your appointment activity. You may receive multiple messages per appointment (confirmation, reminder, form link). Typical frequency is 2-6 messages per appointment.
Message and data rates may apply. Check with your mobile carrier for details about your messaging plan.
You can opt out of text messages at any time by replying STOP to any message you receive from us. You will receive a confirmation message and no further texts will be sent. Opting out of texts does not affect your ability to receive care or use other platform features.

The Moonshot Clinic platform allows you to book appointments, complete intake forms, view upcoming visits, and manage your account. You agree to provide accurate information when using these services.
Appointments may be cancelled through the patient portal or by contacting the clinic. Cancellations made less than 24 hours before the scheduled appointment may be subject to a $50 late cancellation fee charged to your card on file.
Your use of our services is also governed by our Privacy Policy, which describes how we collect, use, and protect your personal information.
The Moonshot Clinic platform is an administrative tool for scheduling, communication, and intake. It does not provide medical advice, diagnosis, or treatment. All clinical decisions are made by licensed healthcare providers during your visit.
Moonshot provides the platform on an "as is" basis. We are not liable for missed messages due to carrier filtering, phone issues, or service interruptions. Important appointment information is also sent via email when an email address is on file.
We may update these Terms from time to time. Changes will be posted on this page with an updated date. Continued use of our services constitutes acceptance of the updated Terms.
