A TRT clinic needs eight software functions: labs-forward charting, e-prescribing with controlled-substance (EPCS) support, lab ordering and result trending, lot-level testosterone inventory, recurring-visit scheduling, membership billing with card-on-file, two-way patient texting, and telehealth. A TRT clinic's software stack is rarely designed. It accretes. You start with an EHR because you need charts. Then a booking tool because the EHR's scheduling is clumsy. Run the audit and most operators find six to ten separate subscriptions: each individually defensible, collectively a mess.
The problem isn't the spend, although the spend is real. It's that none of these tools know about each other, and TRT is a specialty where the layers need to talk: the lab result drives the dose, the dose drives the inventory deduction, the deduction drives the reorder, and the whole chain has to survive a DEA or board audit. So before deciding what to buy, get clear on what each layer actually has to do for TRT specifically, not for a generic medical practice.
1. Charting: labs-forward, dose-forward
Generic EHR charting is built around episodic sick visits. TRT is the opposite, a longitudinal protocol where the chart's job is to show the trend: total and free testosterone, estradiol, hematocrit, PSA over time, next to the dose changes that drove them. If your charting can't put the lab trend and the dose history on one screen, your providers are doing that synthesis by hand at every follow-up.

This is the layer where TRT clinics get burned by generic software. Testosterone is a Schedule III controlled substance under the federal Controlled Substances Act, classified as an anabolic steroid. That means prescribing it electronically runs through the DEA's EPCS framework (Electronic Prescribing for Controlled Substances), with identity-proofing and two-factor authentication requirements for prescribers. Many states now mandate electronic prescribing of controlled substances outright.
The practical requirement: your prescribing layer needs e-prescribing with controlled-substance support, plus routing to the compounding pharmacies TRT actually uses, not just the retail chains a generic e-prescribing tool assumes. 3. Lab ordering and results: the business's heartbeat
A TRT practice is, operationally, a lab practice. Baseline panels qualify the patient, follow-up panels at regular intervals justify the protocol, and safety monitoring (hematocrit, PSA, lipids) is what separates a defensible practice from a liability. Every visit decision hangs off a lab value.

Cadence matters as much as the panel. Common clinical practice confirms a low baseline with two separate morning total-testosterone draws before therapy starts, both documented in the chart. After initiation or a dose change, follow-up labs typically land 6 to 12 weeks out; once a patient is stable on a protocol, monitoring commonly settles into every 6 to 12 months. That rhythm is exactly what your scheduling and recall tooling has to enforce, because a missed follow-up draw is both a clinical gap and lost revenue.
Then there's the logistics of the draw itself. In-house phlebotomy keeps the visit and the draw in one stop but means staffing, supplies, and specimen handling; sending patients to a lab service's patient centers offloads the logistics but adds a step patients skip; telehealth patients need a patient service center near them or mobile phlebotomy. Draw timing relative to injection day also changes how the number reads: a trough drawn just before the next injection tells a different story than a level drawn the day after one, so the order should capture when the draw happened relative to the last dose. All of it points at the same software requirement: a lab interface and scheduling layer that handles where and when the draw happens, not just which panel to run.
4. Inventory: where the spreadsheet dies
If you stock testosterone and administer injections in-office, you are a DEA registrant with recordkeeping obligations: records of receipt and dispensing plus an initial and biennial inventory under 21 CFR 1304.11 (some states require more frequent counts). That means tracking every vial's lot number and expiration date, deducting the exact amount drawn at each injection, and tying each deduction to a patient chart entry.
A spreadsheet fails here for a structural reason, not a discipline reason. The deduction lives in the spreadsheet; the clinical record lives in the EHR; nothing forces them to agree. Every injection is two manual entries in two systems, and every missed or fat-fingered entry is silent drift between what the spreadsheet says is in the fridge and what an auditor counts. Lot-level questions are unanswerable when the lot number was never captured per-dose in the first place, and which patients received lot X? is exactly the question a recall or an audit asks. 5. Scheduling: recurring visits, not one-offs
TRT scheduling is rhythm-based: weekly or biweekly injection visits, lab draws on protocol intervals, provider follow-ups a few times a year. A generic booking tool treats every appointment as an independent event; a TRT clinic needs recurring visit series, visit types with different durations and staff (a 10-minute injection with an MA is not a 30-minute provider consult), and booking rules that respect membership status, so an active member books injection visits freely and a lapsed one doesn't.
6. Billing: memberships and card-on-file
Most cash-pay TRT clinics run on recurring memberships (a monthly rate covering the protocol, injections, and standard labs) with per-visit charges layered on for extras. The billing layer has to do automated recurring charges with card-on-file, handle failed payments with retries and patient notifications (involuntary churn is a silent revenue leak), and tie payment status to clinical operations, so front desk knows at check-in whether the membership is current. A payments platform bolted on beside the EHR can charge cards; what it can't do is answer "is this patient paid up?" inside the workflow where the question is asked. 7. Patient communication: two-way texting
TRT patients are mostly working-age men who do not answer phone calls and do not check patient-portal inboxes. They text. The communication layer needs two-way SMS tied to the patient record for appointment reminders, lab-ready notifications, refill requests, and dose questions, with the conversation history visible in the chart rather than trapped in a separate texting app under a phone number the clinic doesn't control. Refill requests by text are a core workflow, not an edge case: the request should land somewhere a provider can act on it with the chart open.
8. Telehealth: follow-ups, where the rules allow
Telehealth fits TRT naturally for lab-review follow-ups and dose adjustments. Those visits are conversations about numbers, not physical exams. The constraint is regulatory: controlled-substance prescribing via telehealth is governed federally by the Ryan Haight Act plus whatever DEA flexibilities are currently in effect, and state telehealth and prescribing rules layer on top of that. Many clinics land on a hybrid model: baseline evaluation in person, follow-ups virtual. Whatever your model, the video visit should live in the same system as the chart and the labs being reviewed, and you should verify current DEA and state rules before building around a telehealth-first assumption.
The integration tax
Each point solution above is individually cheap and individually fine. The cost shows up in the seams. Patient data gets re-typed at every boundary: intake form to EHR, EHR to lab portal, injection chart to inventory spreadsheet, visit to payment platform. Every re-type is a chance for error and a few minutes of staff time that repeats hundreds of times a month. Worse, no single patient timeline exists anywhere: the labs are in one system, the texts in another, the payments in a third, and reconstructing "what happened with this patient" means opening four tabs. Staff onboarding means provisioning six logins; offboarding means remembering to kill all six.
That said, the math is not one-sided. A solo provider with a small panel and no stocked inventory can run point solutions indefinitely. The seams are annoying, but the volume is low enough that manual reconciliation works, and the flexibility of swapping any one tool has real value. The math flips as the clinic grows: more staff crossing more seams, more injections needing lot-level records, more memberships to reconcile. Somewhere around the point where you stock controlled substances and employ people other than yourself, the integration tax stops being an annoyance and starts being an audit risk.
